Payment
is required at time of service rendered.

by check or cash only.

Any
cancellations must be made at least 24 hours in
advance or clients will be responsible for the full
fee of the session.

Clients
must complete and sign the Client Information and
Release Form. To review and prepare the form prior
to your appointment, you can download them from the
links below. Right click on the link and select
"Save Target As...". Choose where you want to store
the files. You must have Adobe Reader to open the
